  • Self-Management Resource Center - The evidence-based self management programs previously offered by Stanford Patient Education Research Center. Millions of adults live with one or more chronic health conditions. Our programs help people and their caregivers manage their symptoms, improve their quality of life, and reduce healthcare costs.

    Includes Research and Evaluation Tools: With over 20 years of experience developing, adapting, and testing self-administered scales in English and Spanish for research subjects with chronic diseases, SRMC has you covered when it comes to evaluating your programs. These scales are here for you to use in your own research at no cost and without permission, thanks to funding from the National Institute of Nursing Research (NINR). 2018
